We assist with various forms, including but not limited to FMLA (Family and Medical Leave Act) paperwork, disability forms, insurance claims, and other medical documentation.

Is there a fee for completing forms?

Yes, there is a $15 fee for each form we complete. This fee applies to all forms, including FMLA and similar documentation.

How do I submit a form for completion?

Forms can be submitted in person at our office or via email. Please ensure that all necessary sections are completed before submitting the form to avoid delays.

How long does it take for a form to be completed?

Forms are typically completed within 5-7 business days. If you need expedited service, please contact our office for availability and additional fees.

When is the fee for forms due?
